Preschool Census - Planning for Future Enrollment
Dear Parents and Guardians:
In our efforts to plan for and provide quality instruction to all children who reside in Prairie Grove District #46, we are looking to gather information regarding children who will eventually attend school at District 46. Each year students who enter kindergarten do go through a registration process. However, we are not always aware of children ages 1-4 who will at some time begin to attend school. The attached link is a preschool census form. We encourage parents/guardians to complete a form for each child, age 0-5, who resides in your household. This information will be utilized for enrollment data so we can plan effectively for class sizes and staffing needs.
All information collected will be strictly confidential and only utilized for enrollment purposes for District 46 and child find requirements as they pertain to early childhood education as outlined by the Illinois State Board of Education.
You may download the census in PDF form and mail it or drop it off at the district office or you can download the form in Microsoft Word format, fill it out electronically and submit via Email.
